When you make your first appointment at Pinewood Healing Arts, we ask that you fill out some forms for the registration process.

• Medical Records Release and Statement of Financial Responsibility

To expedite your first visit at Pinewood Healing Arts, download and complete the appropriate paperwork and bring it along with a copy of your referral and insurance coverage, if needed. If you are unable to complete your forms, please plan on arriving 10-15 minutes early to complete paperwork.
